It was determined that a child was left unsupervised in a classroom for approximately 6-8 minutes.

At the time of inspection the operation was transporting children without required admission information indluding parent contact and medical authorization. This was corrected at inspection by staff making copies of the children's admission/medical authorization forms and adding to a transportation binder.

At the time of inspection 10/10 staff did not have any of the required annual health/safety trainings.

At the time of inspection one child with a diagnosed food allergy did not have an emergency food allergy response plan.

At the time of inspection the operation did not have a child w/ diagnosed food allergies posted. This was corrected when operation staff updated the food allergy list to include the child's name and allergy information.

One child's immunization record was missing.

At the time of inspection there were no written feeding instructions available for 2 infants in care.

This standard was evaluated based on a reported allegation. The evidence shows that the director did not demonstrate self-control in the presence of children. The director engaged in an escalated verbal altercation with a parent in the presence of children.

10 child files were evaluated. Three children did not have current shot records on file.

The toddler room was observed during naptime. There was a 16 month old in the classroom. There was a total of 16 children in the classroom. The specified age group was 2 years old. The classroom was over ratio by 5 children. The director added a 2nd caregiver to the room immediately.

Based on evidence gathered from a DFPS investigation, it is determined that a caregiver became frustrated with classroom conditions which resulted in the caregiver lifting an 18 month old off the ground by one arm causing bruising to the child's left arm.

Based on information obtained in regards to a DFPS investigation, it is determined that caregivers are placed in classrooms and do not have the skills, abilities, or training to adequately manage the classroom. This is evidenced by caregivers being overwhelmed.

This standard was evaluated in regards to a DFPS investigation. At least one class at the operation does not have an activity plan that is followed. The children are allowed to "free play" all day.

An infant caregiver did not use good judgment when she slapped infants on the hands as a form of discipline. A second caregiver did not demonstrate good judgment when she pulled children by the arm and handled children in a rough manner. This information was obtained through an abuse/neglect (DFPS) investigation.

This standard was evaluated in regards to a DFPS investigation. An infant caregiver inappropriately disciplines the children by slapping them on the hand.

Based on evidence gathered from a DFPS investigation, it is determined that caregivers did not exercise good judgement by failing to report inappropriate discipline and mishandling to children by a co worker. Caregivers stated that they witnessed this concerning mishandling on multiple occasions, and did not report their concerns.

This standard was evaluated in regards to an investigation and is found to be deficient. The center director failed to report child safety concerns to Licensing after being informed of the allegations.

At time of inspection, two nap mat covers had cracked corners. The corners were plastic and had sharp edges. The cracked plastic also posed a pinching hazard. There were two tricycles on the playground that were broken. One tricycle had a broken pedal and the other had broken plastic pieces with sharp edges. Photos were taken of all equipment. The center director removed all broken equipment during inspection.

A child with a diagnosed food allergy and a prescribed epi pen, did not have a food allergy emergency plan accessible in his file during inspection.

The operation's last health inspection was conducted on 11/5/2020.

At the time of inspection, 1 child did not have a copy of healthcare recommendations or orders for specialized medical assistance (g-tube feeding) by a health care professionals. The child did not have a health care statement completed by a physician on file.

At the time of inspection, there was a 2.5 yr old child in the infant room with an unspecified medical condition. It was explained that the child was non-mobile and required a feeding tube. Caregivers stated that the child participates

Ten children's shot records were evaluated. One child did not have a shot record on file. Five children had out of date shot records on file.

Caregivers did not display the necessary skills when dealing with children with difficult behaviors.

A caregiver strapped a 3 year old child in a stroller as a form of discipline.

2 out of 10 children did not have a copy of shot records on file.

3 out of 10 children did not have a health statement on file.
